Our Fees:
Our goal is to provide you with the best possible care at a reasonable cost. Our fees will depend on the type and extent of treatment you need and are comparable by other ears, nose and throat specialists. If you have any questions about our fees, please be sure to discuss them with us during your visit.
Our Payment Policy:
We ask that you pay for all office services at the time of your visit, unless we participate with your insurance plan. If we do, please pay at the time of your visit any deductible amount, co-insurance, or other balance not paid by your insurance company.
Your Insurance:
We participate with several insurance plans, HMO's and PPO's. We will be happy to answer any questions you may have about our participation with your particular insurance program.
If your insurance requires a referral, please ensure that we have a referral prior to your office visit.
Extra insurance forms and special letters to insurance companies and attorneys might necessitate an additional fee, due to the considerable amount of secretarial and physician time needed to prepare them.
